Southeastern Integrated Care is truly an integrated model of care, contracted with our regional MCO’s and licensed by NC DHHS to provide a continuum of enhanced behavioral health services, primary care, and soon to be residential services.  We believe our integrated care delivery system which also incorporated chronic care management, collaborative care management and remote patient monitoring, allows for the easiest access of care to treat the whole person, while assuring all clients in need of care have a “door of entry” through primary care and/or behavioral health care to get their healthcare needs met with dignity and honor. 

Along with our integrated model of treatment care, we realized that removing the social determinants to healthcare are critical. Therefore, our non-profit, Southeastern Life Center, focuses on assuring the basics too care are provided including transportation, nutrition, medication assistance and education. 

Our peer support teams and services embedded throughout our delivery system alongside our community outreach team members assure that we meet our clients where they are at, wherever they are at, and keep them engaged in the recovery process. We realize that our “sticktoitiveness” approach to care, allows for the best opportunities for the clients we serve to receive the support and encouragement they deserve as they successfully battle the brain diseases of behavioral health and substance use disorders.
